Program Requirements
- Minimum of 5 enrolled employees to activate corporate pricing
- Membership options available for both employer-sponsored enrollment and individual employee-pay arrangements
- Employees may add their own family members at the additional Family Membership Plan rate
- Each employee enrolls directly with APEX — simple and straightforward
